HealthPartners - Medication Therapy Management Services

The goal of HealthPartners Medication Therapy Management (MTM) program is to optimize therapeutic outcomes for targeted Medicare Part D Beneficiaries and Minnesota Health Care Programs (GAMC, MA, MinnesotaCare, MSHO, and Minnesota Senior Care) recipients through improved medication use, cost-effective medication use and reduction of adverse events, including adverse drug interactions.

HealthPartners has a network of pharmacists who are providing Medication MTM services for its eligible members in accordance with program standards. During 2010 pharmacists will meet one-on-one with eligible members to perform a comprehensive MTM review to optimize therapeutic outcomes. The reimbursement rate for the MTM program will range up to $150 per eligible member, which includes pre-visit planning, a face-to-face MTM consultation, and post-visit services. MTMPath™ software will be provided free of charge to participating pharmacies for performing these MTM services as a member of the Medication Pathfinder Network™. The web-based software can also be used by the enrolled pharmacist to perform MTM services for no additional cost -- on any patient, on behalf of any payer -- as long as he or she continues participating in HealthPartner's MTM program. Retail, clinic and consulting pharmacists are welcome to join the network if the credentialing criteria are met.
