Aren't the insurance companies already doing this?
There are many patients who do not fit the Centers for Medicare & Medicaid Services (CMS) criteria for MTM but can benefit from your consulting services. We can assist you to identify these patients so that you can offer your services and build a proper structure for a fee-based consulting practice.

What is the cost of becoming part of the Medication Pathfinder Network?
There is a software license fee that is paid monthly and an annual per patient storage fee. Please contact Medication Pathfinder for a complete pricing structure.

What hardware and software are required?
You’ll need a basic computer with internet access (preferably broadband), any web browser, Adobe Acrobat Reader 7.0 or higher (available for free by clicking here: download Adobe Acrobat reader), and access to a scanner or fax machine.

Is this program secure and in compliance with HIPAA regulations?

In addition, our CE programs provide practical skills that build on your clinical expertise to assist in creating a successful MTM program.

How will my patients benefit from MTM?
MTM can be a very valuable service to your patients. By performing a comprehensive drug and medical review, you can help detect, resolve and prevent medication-related issues. What's more, you can increase your patients’ education and understanding of their medication and enhance patients’ medication adherence.

Your patients will feel safer and more secure about their drug regimen when you review it with them. You can save your patients money by streamlining their medication usage to find the most cost-effective, medically appropriate drug regimen. In addition, they will also have access to PMR Smart, their own personal portable medical record.

How much time does it take for a Medication Pathfinder patient session?
Depending on a patient’s medical history, a typical session lasts between 15 and 45 minutes.

Can I bill Medicare Part D insurers for this service?
MTM Path produces HCFA 1500 bills using AMA billing codes for patient and insurance purposes.

I have patients in a long term care facility. Can I offer them medication therapy management services?
Yes. In fact, you’ll find these services are sought and welcomed by the staff in long-term care facilities.

Who can enter data into MTM Path? Does it have to be a pharmacist or can a technician enter data into the system?
Every enrolled pharmacist has his/her own account. The software is very simple to use; once the lead pharmacist on the account has been through the procedure at least once, the determination can be made about additional staff members who can input the data.

I want to perform MTM services, but I don’t have the time to input data or make recommendations. What can I do?
The Medication Pathfinder Network has trained pharmacists on staff who can enter information for you and/or make recommendations for a small additional per-patient fee. The fees can be structured on a cafeteria-type basis so that you only pay for the services you need to utilize.

What kind of marketing support does Medication Pathfinder offer?
We provide marketing materials such as brochures and posters to help attract MTM participants to your pharmacy. We can assist you in contacting your patients who are candidates for MTM consulting services

What kind of training will I receive once I enroll?
The Medication Pathfinder Network offers ACPE accredited live CE programs, web seminars, technical support, and a training manual with step-by-step instructions.

What is PMR SmartTM?
PMR Smart is a web-based personal medical record on demand that your patients can access wherever they go. The record is maintained on a secure server and can be accessed from medical offices, emergency rooms, hospitals – anywhere in the world on a machine with an internet connection.

How can my patients access their PMR Smart record?
Once enrolled, all participants will be emailed a login and password to enter PMR Smart.com.
